Named Accountable GP for All Patients

Every patient has been allocated a named GP who is responsible for your overall care at the practice. If you would like to know who your named GP is, please contact reception and they will be happy to inform you.

If you wish to change the name of your accountable GP, please let us know and we will make every  reasonable effort to accommodate this request.

Having a named accountable GP does not restrict your entitlement to see any doctor in the practice.

To ensure that you obtain the latest advice and appropriate vaccinations for your destination we ask that you visit one of the following websites PRIOR TO BOOKING AN APPOINTMENT and obtain a printout (to bring with you) of which vaccinations are recommended:

Before booking an appointment, please download and complete one of these Travel assessment Forms, it will enable our nurses to ensure you receive the correct services.  Once completed please hand the form in at any of our surgeries.  The form is also available to pick up at any of our surgeries.

We keep a limited supply of the following travel vaccines at the surgery and are happy to administer these, free of charge, if appropriate:
  • Hepatitis A (Adult and Junior)
  • Typhoid
  • Tetanus/Diptheria/Polio
