New Patient Registration
We accept new patients who live within our practice area. As it often takes sometime for records to be forwarded from your former practice all newly registered patients will be asked to complete a health questionnaire and are offered a consultation with the doctor.
Please allow our staff 5 working days to process your registration before requesting an appointment.
We are using a new online service called Register with a GP surgery that makes it easy to register with this GP surgery.
Just fill in this quick online form to start the process. You do not need proof of address or immigration status, ID or an NHS number.
The service is designed and run by the NHS, so your personal information is safe. It cuts our administrative workload and makes it easier for you to register.

Paper forms are still available from any of our reception desks if you need one.
New Patients must complete the following online form and a separate additional information forms. One for all patients aged 13 and over and one for children aged 12 and younger. Please hand the additional information form to a receptionist on your first appointment.

We also request that new patients bring some identification which will allow us to accurately record your details on our system. A photo-card drivers licence would be great as it shows both your name and address. We understand that this is not possible for everyone and we will register you even if you are unable to provide identification.
